OHT: ConnectMyHealth - New Digital Tool

The Ontario Health Teams in Ontario Health West Region (OH West Region, also known as Southwestern Ontario) is promoting a online digital health records system. ConnectMyHealth is a free digital health solution that provides patients with an online, single access channel to view their health records from participating hospitals.

ConnectMyHealth pulls various types of health records from participating OH West Region hospitals into one, consolidated view; this is as opposed to patients having to access multiple systems to access their records from participating hospitals.

What does ConnectMyHealth include?

• Record sharing

• New results alerts

• Patient dashboard

• Patient visit timelines

Who can register for a ConnectMyHealth account?

• Patients who meet the following criteria can register for their own account:

  • Be 16 years of age or older

  • Have a valid Ontario Health Card

  • Have a unique email address (not shared with anyone else)
