
Aging in Place - Huron
Aging in Place - Huron is a pilot program focused on understanding the experiences of older adults living in Huron County. The first phase of the program is a community survey that will explore the experiences and challenges older adults face when aging at home. The information gathered will help identify local needs and guide the development of future initiatives that
support healthy, independent aging.
Project Background
Huron County's older adult population continues to grow, increasing the need for community based supports that help people remain healthy, independent, and connected as they age. With ongoing healthcare challenges, including limited access to services and rural healthcare workforce shortages, there is growing pressure on our healthcare systems. This pilot program was inspired by the World Health Organization's Integrated Care for Older People (ICOPE) framework, which recognizes that healthy aging extends beyond medical care. The framework promotes an integrated approach to care that supports mobility, cognition, vision, hearing, nutrition, and mental well-being, helping older adults maintain their independence and quality of life. Through this pilot, Gateway CERH aims to better understand the experiences and needs of older adults in Huron County to inform proactive initiatives that encourage aging in place for as long as possible.
